I found the link you provided very helpful, however i only have a small collection of domains and was hoping there was some way of making a up a FORM in Frontpage?

Look at your menu bar.
Insert > Form
There are many options for you to chose from.
Once you have inserted the form, you can modify its properties by right clicking on the form > Form Properties
