It's very simple. Just use a free tool like this to generate a favicon.ico file and then load it up to the root directory of your website.

Whether it adds value to use keywords in image/file names, I am not sure, but I do it anyway!
I also use .gif image formats when generating my favicon in order to make the image background tansparent so it blends into each browser vs. displaying a white square on non-white browser backgrounds.
I have an example of this on the Free Tulip Pictures site linked below. The Orange Tulip favicon stands-alone against the background of any browser colour (which is sharper, imo, then having a white box around it).

Maybe if you have a CMS it's in there as a disseminated call-object (ie: seperate file), as I have a php cms script I bought and each file is seperated (header, footer, sidebar, nav, footer, etc), so I have to go to the seperate file to update it vs. the index file.

The site is running phpMyDirectory. I've checked the files in the root directory (index.php etc.) as well as the one in the template. :(
The site looks a mess, too. When I first set it up I just changed a few colors and the logos at the top. Now, I've added some adsense, changed the pictures to be better and contacted my wife's cousin about a logo.
Edit: How the hell did I miss it? It was in /templates/(template)/header.tpl
