needed quick help with php members database

I had a great site MMACams.com that is now down. Its a PPV webcam service which i use a third party to do the actual ppv. My server shut me down and I lost everything. I need to quickly get this rebuilt. It should take less then a day. I do need a simple php members database installed. I emailed the owner of the third party company and he said to write this on the form..

On the login.php page to your site you will have code that contacts our gateway api (user_create) right after you verify the member's credentials with your local db. At this point both our databases are in sync and you only need to set the four session cookies for the player to work. That's it

Yea. You need to create a php login form backed up by a database, that sets the relevant cookies required.
