The fact that there appears to be no upstream filtering really bugs me - the providers appear to be doing nothing to help their customers to avoid the plague of spam that now infects the Internet.
My provider BT in the UK recently announced new anti-spam measures - net result almost every email comes with {Spam} denoted on it and the message opt-out to confirm good mail appears not to work.
I am now using two filtering programmes to remove the garbage from my email - the Internet stinks from this garbage and everyone is affected.

That's basically the reason why there's spam, isn't it? The spams are meant
for the 1% or less who'll order from them.

Regarding your question about an "international coordination" to discussing
this, there is at least one. (somewhat, anyway...)

US-based Internet marketers are facing a new dilemma: depending on the
DMCA and the FTC's ongoing "research", legitimate ones might eventually be
asked to keep a "do not email" list for those who successfully opted out of
their mailing lists. Imagine the nightmare of doing such aside from the already
damn growing amount of spam.
