A combined contact form/newsletter script?

I'm looking for a combined contact form/newsletter script - i.e. one script that has both functionalities.

A visitor can use the form to contact the webmaster and, optionally, subscribe to the newsletter at the same time as well. Or just use the form to subscribe. The script must have the normal newsletter features and use either a MySQL or flat file database.

Free would be nice, but a cheap script would also make me happy. PHP preferred.

I'm pretty sure I've seen this somewhere, so don't really want to pay for custom-development. Just thought I'd tap into the vast knowledge and experience of the dnForum users ...
