Membership Proxy Script

Hi,

Do you know any proxy script that requires the users to sign up before they can surf using the proxy? I need such a script for my proxy site, because I want to prevent that users abuse my proxy to hack other websites, etc. Maybe something with IP recording and/or email verification.
