I remember trying to view my stats from a adult affiliate site I use, and I couldn't access the page, Nothing pornographic to see, just stats.
Or is your point that an "effective" porn filter is one which prevents viewing a site where there is "nothing pornographic"?
We had a nitwit IT guy that set up our server to block anything with "sex" in the URL, which was so beyond brain-damaged. It was really "effective" at blocking my access to a map to a courthouse in Sussex County.

People set blocks and filters on "known" search terms. The webmasters of these porn sites are handsomely compensated to get around blocks and filters simply by altering their scripts or adding/changing keywords and metatags.
I recall reading a piece about the Columbine shootings. The Webmasters are so adept at their craft they were able to divert a great deal of traffic to their porn sites.
I have no idea what they were capable of doing or which words were selected to be their "trigger words". I do recall one time searching for tickets to Bill Cosby, War Memorial, Greensboro or something to that effect. I ended up on a Porn Site that immediately started downloading something (spyware? virus?). The only way I could get out of the site was to disconnect the power and reboot.
