I believe that is the format when no account id is found. Check your code. It may have your account id missing.

I basically copy the same code from the header part of my page where the ad is displayed normally. So same code, different locations: one is normal, one is premium look.
And my site's traffic is nowhere close to premium publisher's requirement.

In the picture there's no "Ads by Google", you might get into trouble because of that. By the way, what's the website? Maybe the problem is not in the add itself...

I have tried that once when my Adsense code has missing texts. My sites' number of visitors are hundreds of thousands away of being premium.

looks more like a bidvertiser or revenue pilot ad. If it's a google ad something is wrong with the code display.

Yes, this occurs when something's wrong with the code (experienced it myself, so looked into it). Maybe something is wrong and you just don't know about it? Could it be your CMS that's screwing with the code (that's what happened to me back then)
Don't check the code before it is inserted ('cos that's just copy/paste), check it in "view source" when the page is live.
