the vast majority of search engines couldnt care less what the extension is when indexing.
However (there is always that annoying however), some localized search engines will not index a .us site (which to a certain degree makes sense).
but this is a small minority.
I agree that you should get both, and if the .us keyword is much better, develop that and forward the traffic of the .com to the developed .us site

Depending on the keyword, it has also been rumored that Google (in it's efforts to improve search and fight spam) advances .us extension names. One reason being the relatively large % of these sites that are "official" as in the States, Cities and Towns etc.
