can anyone create me a small program i am willing to pay that can do this below

I have a list of words eg below in txt format

Mansfield
Test
Notts
luck
bolsover

I want to import this txt file so i get the following below Bold is what i need it to replace the info in the txt file also in Bold is ato number there could be as many as 1000 words in the txt file to import.


INSERT INTO `rl_categories` VALUES (1, 'Mansfield', '', 0, 0, 1, '', '', 1, 0);
INSERT INTO `rl_categories` VALUES (2, 'Test', '', 0, 0, 2, '', '', 1, 0);
INSERT INTO `rl_categories` VALUES (3, 'Notts', '', 0, 0, 3, '', '', 1, 0);
INSERT INTO `rl_categories` VALUES (4, 'luck', '', 0, 0, 4, '', '', 1, 0);
INSERT INTO `rl_categories` VALUES (5, 'bolsover', '', 0, 0, 5, '', '', 1, 0);

This could be done using PHP or better yet a mySQL script with LOAD DATA INFILE function (if mysql user has FILE privilege). Feel free to PM me.
