If I want to register a .ca extension and I know someone in canada how does that work? Do they register it and push it to me? I am confused on how the process works.

You need to be a Canadian national or resident. Or a local company or have a TM, plus a few eligible categories of individuals such as the Queen of England
You could ask a friend there to register the domain on your behalf but he/she would be the legal owner.
Well if you are not eligible, you may not own one.
You need to have a trusted person to hold ownership of the domain name. That itself is circumventing the rules, it's not endorsed or recommended by the registry either.
You'd need to know someone with Canadian presence you could trust to own it for you. Legally they would own it, so you'd have to trust them very well to do what you wanted with the domain.
