Database wanted

Hi all.

I am looking for a simple access database for all the people i contact when i email or post customers about the domain names i have, where as i can input the follwoing fields.

Name
address
email address
web address
date emailed
date replied
reply comments
names emailed ( 5 boxes 8 characters long each )
notes

Is there anyone here that can do this.
Would it be free or chargable.
