If you're looking for self-managed and not too bothered with quick support, then Leaseweb (in the Netherlands) can bee a good choice. Good value, especially if you're looking for high bandwidth. Support is good, generally, but response times vary a lot - from minutes to several days, so they aren't the best if you're not a good server admin yourself.
The company I use most is in the US, Liquid Web, and are top notch in reliability and support, and work pro-actively to keep your server tip-top. I have several servers with them, and use them for all services which I can't afford any down time for.
