Wanted: Service Help in filling W8BEN DomainSponsor form

Who can help me, please ?
I need to fax back the W8BEN form to DomainSponsor, here're more details and what I need to know exactly:

- I'm a foreigner individual (Italian).
I filled Part 1 with my postal address and I identified myself as "Individual" checking only this option.
I need to know: what's the reference number requested on point 8 ?
In Part 2 I should only specify that I'm a resident of "Italy" on point 9a without checking any of the other options, correct ?
Now, what else should I fill in, among all the following points (apart signing the form and entering the date of the day, of course) ?
What should I enter in the field "Capacity on which acting" ?

I was wondering the same thing, got filled out the 1 part, but don`t know what do but into 2, 3 part.

For my understanding, the real problem is that, if they send out next check before they got this form, they'll deduct a 31% commission from our revenue, is this correct ?

Not sure they'll do it for the month of August too but better not risking, hopefully someone will help us,I've just forwarded my questions to their billing department as well.

Complete part 1 as an individual.
No reference number required in 8.
As you say in part 2, only complete 9a - nothing else.
Don't fill in anthing else.
Sign and date -- your capacity is "as individual".

Stuff .. I have filled out a few of these for my Canadian Corp. Here is how I do it.;

In Part 1
1. Fill in the Name of your corp or organization.
2. Fill in Coubtry of Incorporation or Organization
3. Check off type of Beneficial owner
4. Complete address of Corp or Organization
5. Only if mailing address is different than #4

Part II
9. You "should" choose "a" and fill in your Country name.

Part IV

Sign your Name, Date and your Title.

That should do it.

Make a note directing it to the "accounting dept" when faxing to Oversee.net (Domainsponsor.com).
