Site Requirements:
All that will basically be needed is a decent VPS/Dedicated server. I currently use a VPS with 512MB ram and 600GB bandwidth and have no problems.
Revenue Details:
Site Unlocker earns about 300$/mo, it's revenue from two sources, Google AdSense and Adversal (pop-unders). The AdSense ads can be seen on the home page and the pop-unders will appear on all of the proxified pages.

As you can see, the traffic is primarily type in, although it gets a very decent amount of search engine traffic as well. The MySpace traffic is from when I had some friends of mine post some bulletins, it also comes from MySpace user's profiles as I know some users have linked to it.
