Can a registered trademark "A" be transferred/sold to another entity that owns registered trademark "A2" - very similar to "A" ?

If so what's the best approach?

Yes.

I'm not sure I understand the question.

If you mean "how can one party transfer ownership of a registered trademark to another party?" then it can be done online here:

http://etas.uspto.gov/

To maintain rights in mark "A", then the assignee will have to continue to use "A" on similar goods or services.

Thank you John.
