Advice on domain that vanished from my account

I need advice to get back a domain that I found vanished from my account yesterday. My registrar told me it was transferred to another registrar, with the administrator's (me) permission, but I have received no emails requesting authorization.

The strange thing is that transfer could not have happened as the domain has not passed the compulsory 60-days lock period before it can be transferred out.

Anyone, any thoughts what might have happened and what should I do?

pm me the domain name, and the registrar name with whom u had this domain.
Also when did u check it for being in ur account last time?
