Wanted: Service anyone used the Moniker ESCROW??

I have a problem....I have started the deal on Moniker...
and also got the reply from them....now I need to sign and fax to them...
but I don't know where I need to sign.....

I have upload the two pic data...and have some problem for HELP...

1. the title...."To: (Seller/Buyer please fill in your contact information)
what data I need to fill?? behind the "To:"??
2. the total payment I need to send is the price list on "Total payment due on receipt".....the (Wires – add $20 for Domestic or $25 for Foreign) is for the bank???

3. I am the buyer...so I only need to sign "the buyer"...right?
and the "sign" and "name" have different?or fill the same??

if you ever used the system...please help me...
