Cash / Trade & Buy / Sell Agreement Form

Anyone have a copy of a cash/trade buy/sell domain agreement form that I can use?

I'm selling a domain for cash and accepting a trade as part of the deal. It doesn't have to be perfect, as I can modify it. Anything would help! I've never seen an agreement for this type of transaction.
