Domain Registrar Unacceptable Error source?

Hi,

I came across this thread today where an account from Network solutions fell foul to Social engineering attack and customer lost access to his account and relative services like email, website, domain.

Which got me thinking and hence the question is there a source for unacceptable mistakes (stolen domains, security breach) committed at registrars?

I do not mean delayed customer service response etc.

It would be good to know whom not to register with.
